Anti-Saccaromyces cerevisiae antibodies (ASCA) seem to be associated with Crohn’s disease (CD), while anti-neutrophil cytoplasm antibodies (p-ANCA) seem to be a recognised marker for ulcerative colitis (UC). ASCA positivity has been found to be associated with a deficiency in mannan binding lectin, a component of the innate immune system. The theory that the initial incident leading to IBD is an infection in patients with a defect in their innate immune system is still speculation. Background/aims: The measurement of perinuclear antineutrophil cytoplasmic (pANCA) and anti-Saccharomyces cerevisiae antibodies (ASCA) has recently been suggested as a valuable and noninvasive diagnostic approach in the differentiation of ulcerative colitis (UC), Crohn’s disease (CD) and indeterminate colitis (IC). Clinical Use ASCA is significantly more prevalent in Crohn’s disease than in UC or in healthy individuals. A positive ASCA test is a useful adjunct to other clinical and laboratory findings to establish a diagnosis of Crohn’s Disease.
